Job summary

Maximus is seeking a Registered Nurse, Clinical Assessor to join our team. You will conduct person-centered assessments in homes, group homes, and care facilities to determine Long Term Care needs and placement options, in accordance with contract directives.

This role requires travel within Cumberland & York counties, and may involve on-site, video, or telephonic assessments. We support flexible schedules, competitive pay, and a full benefits package.

Qualifications

• Minimum two (2) years of post-graduate nursing experience.
• Specific clinical license required per contract requirements.
• Travel to client locations for more than 50% per workweek.
• Experience in long-term care or home health care preferred.
• Experience with OASIS or MDS preferred.

Responsibilities

• Complete medical and behavioral health assessments using standardized tools and criteria.
• Conduct on-site, video, and telephonic assessments according to program requirements.
• Ensure accurate, HIPAA-compliant documentation of assessment outcomes.
• Travel to client sites to support program delivery more than 50% of the workweek.
