Job summary

Maximus is seeking a Registered Nurse to serve as a Clinical Assessor in Cumberland Center, ME. This hybrid remote role requires performing person-centered assessments in homes, group homes, and care facilities to determine LTC needs and placement options.

You will commute within Cumberland & York counties and support multiple programs under the State of Maine contract. Eligible candidates have RN licensure, 2+ years nursing experience, and familiarity with long-term care or home health care.

Qualifications

• Active RN license in the United States.
• Two years post-graduate nursing experience required.
• Willingness to travel within Cumberland & York counties (>50% of the week).
• Experience with long term care or home health care preferred.
• Knowledge of OASIS or MDS is a plus.

Responsibilities

• Complete medical and behavioral health assessments using standardized tools per contract requirements.
• Conduct on-site, video, or telephonic assessments to determine needs and level of care.
• Document assessment outcomes accurately in HIPAA-compliant records.
• Travel to client sites to perform in-person assessments as needed.
• Coordinate with senior staff on complex cases and contract directives.
