Acentra Health is seeking a Clinical Assessor to join our growing team in North Carolina. This hybrid role covers PCS and CAP assessments for Medicaid beneficiaries, with travel within a 60-mile radius and assessments typically conducted in the beneficiary's home or community setting.

Qualified candidates hold an RN license (NC or Compact) and have 2+ years of nursing experience. Strong communication, privacy, and case-management collaboration are essential to succeed in this role.

Qualifications

• Registered Nurse (NC or Compact license) is required.
• Minimum of two years of nursing experience.
• Travel up to a 60 mile radius required.

Responsibilities

• Conduct PCS/LOC assessments to determine eligibility and level of care.
• Ensure PCS services are provided on a needs basis.
• Maintain privacy and dignity of beneficiaries during assessments.
• Meet established timeframes for new, expedited, annual, and change of status assessments.
• Interview family members and informal caregivers during assessments.
• Provide Beneficiary guidance to select PCS providers and assist with service plan reviews.
