Acentra Health is seeking a Clinical Assessor in Wake County, North Carolina for a hybrid role. The position involves field work and requires an RN license with two years of nursing experience.

You will conduct assessments for PCS eligibility, ensure appropriate service levels, maintain privacy, and support beneficiaries and families through the process in collaboration with state requirements.

Qualifications

• Registered Nurse license (NC or Compact) required.
• Minimum of two years nursing experience.
• Travel up to a 60 mile radius.

Responsibilities

• Conduct assessments to determine PCS eligibility.
• Ensure PCS are provided on a needs basis.
• Maintain privacy and dignity of individuals.
• Adhere to timeframes for assessments and reviews.
• Interview beneficiaries, families, and caregivers.
• Guide beneficiaries to select PCS providers.
• Participate in service plan reviews and case consultations.
• Submit assessments using state-approved interface.
• Assist in mediation and appeals processes.
• Respond to state inquiries regarding assessments.
