Qualifications

• Valid RN license (NC or Compact) required.
• Minimum of two years nursing experience.
• Travel up to a 60 mile radius.
• Hybrid role based in Cabarrus County, NC with field work in the region.

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved tools.
• Ensure services are provided on a needs basis according to unmet needs.
• Maintain privacy and dignity of beneficiaries during assessments.
• Complete initial and annual LOC assessments for CAP/PCS participants.
• Assist beneficiaries in selecting PCS providers and conduct service plan reviews.
• Engage in mediation/appeal processes and respond to state inquiries.
