Job summary

Acentra Health is seeking a Clinical Assessor in North Carolina to perform needs-based eligibility determinations for PCS and CAP beneficiaries. The role is hybrid, based in Cabarrus County, with travel to assessment locations in your region and work from a home office.

You will complete LOC assessments, coordinate with families and case management, ensure privacy and timely reviews, and use state-approved tools to determine eligibility for Medicaid-funded services.

Qualifications

• RN license in NC or Compact.
• Minimum of two years of nursing experience.
• Hybrid position based in Cabarrus County, NC.
• Travel up to 60 miles radius for field assessments.

Responsibilities

• Conducts needs-based PCS/LOC assessments using state-approved tools.
• Ensure PCS/CAP services are provided on a needs basis.
• Maintain privacy and dignity of beneficiaries during assessments.
• Submit completed assessments via state-approved interface.
• Collaborate with family members and informal caregivers.
