Job summary

Acentra Health is seeking a Clinical Assessor to join our team. This role conducts needs-based eligibility determinations for NC Medicaid Personal Care Services (PCS) and CAP waivers, typically in the beneficiary's home or community setting.

Travel within a 60 mile radius from your home base is required, with a hybrid work arrangement in North Carolina. Qualified candidates are RNs or LCSWs, with at least two years of nursing experience, and comfort with using Microsoft Excel, Word and Outlook

Qualifications

• Registered Nurse or LCSW license by the applicable state.
• Minimum of two years of nursing experience.
• Travel up to a 60 mile radius.

Responsibilities

• Complete needs-based eligibility determinations for NC Medicaid PCS.
• Perform LOC assessments for CAP/PCS programs to determine level of care.
• Travel to assessment locations within the region and work from home base.
