Job summary

Acentra Health is seeking a Clinical Assessor to join our growing team. Under the PCS and CAP programs, you will perform needs-based LOC assessments for NC Medicaid beneficiaries, determine eligibility, and help families plan services delivered in home or community settings.

This hybrid role requires travel within a 60-mile radius of your North Carolina home base, with preference for Cabarrus County (Concord and surrounding areas).

Qualifications

• Registered Nurse license (NC or Compact) required.
• Minimum of two years of nursing experience required.
• Willingness to travel up to 60 miles radius within North Carolina.

Responsibilities

• Conduct needs-based LOC assessments to determine PCS eligibility.
• Perform assessments and ensure PCS services are provided on a needs basis.
• Interact with beneficiaries, families, and providers to guide service planning.
• Prepare and submit assessments using state-approved interfaces.
• Participate in CAP/PCS care coordination, mediation, and appeals as needed.
• Adhere to HIPAA and privacy/security requirements.
