Madison Healthcare Services Clinic is seeking a qualified Registered Nurse to join our team in Minnesota. Enjoy rural experience with urban benefits and a schedule of 64–80 hours per pay period.

You will deliver patient-centered care, support care coordination, clinic triage, and nursing informatics initiatives. Responsibilities include direct patient care, coordination of referrals, and collaboration with clinic and hospital teams to ensure accurate documentation and quality outcomes.

Qualifications

• Current Registered Nurse (RN) license in the State of Minnesota required.
• Associates degree in nursing required; bachelor’s preferred.
• Experience in clinic and hospital settings is a plus.
• Five years of nursing experience preferred; care coordination or informatics desirable.

Responsabilités

• Provide direct patient care across clinic teams.
• Coordinate patient care, referrals, follow-up, and communication among patients and providers.
• Perform clinic triage by phone and in person using established protocols.
• Support nursing education and training activities across clinic and hospital teams.
• Assist nursing informatics initiatives and EPIC documentation to improve workflows.
• Collaborate with LPNs, CMAs, and hospital staff to ensure safe, high-quality care.
• Participate in chronic care management and quality improvement activities.
