Job summary

NorthLakes Community Clinic in Wisconsin seeks a Registered Nurse for triage by phone, providing health guidance using evidence-based protocols and handling prescription refill requests. The role requires residency in NLCC's service area at hire and throughout employment.

Responsibilities include nursing assessments, following clinical guidelines, coordinating care with ED and specialty providers, and educating patients about community resources.

Qualifications

• Associates or Bachelors in Nursing required.
• WI RN license and 2+ years of related experience.
• Excellent communication and counseling skills; local knowledge of community resources desirable.

Responsibilities

• Perform nursing assessment and triage by phone per protocols.
• Complete refill requests for non-controlled substances per NLCC guidelines.
• Complete refill requests for controlled substances under provider direction.
• Coordinate ED and hospital follow-ups and connect patients to community resources.
• Document in the EHR and participate in quality improvement efforts.
