Job summary

Madison Healthcare Services’ Clinic seeks a qualified Registered Nurse to join our team, providing patient-centered care and coordinating clinic operations. You will work with clinic and hospital teams to promote continuity of care and efficient workflows.

Requirements include an Associate degree in nursing, Minnesota RN license, and 5 years of nursing experience preferred; bachelor’s degree preferred. EPIC experience is desirable, with a focus on quality outcomes.

Qualifications

• Associates degree in nursing is required; a bachelor’s degree is preferred.
• Active RN license in Minnesota is required.
• Experience in clinic and hospital settings is preferred.
• Five years of nursing experience is preferred; care coordination, education, triage, or informatics experience desirable.
• Ability to work independently and coordinate activities across departments.
• Strong assessment, reporting, and documentation skills; EPIC experience preferred.

Responsibilities

• Provide direct patient care across clinic care teams as needed for patient needs, staffing, and clinic operations.
• Coordinate patient care, referrals, follow-up, and communication among patients, providers, and care teams.
• Perform clinic triage by phone and in person; assess concerns and determine next steps within RN scope.
• Coordinate clinical staff education and training activities for clinic and hospital teams.
• Support nursing informatics, EHR workflows, documentation practices, and process improvement.
• Utilize EPIC and other systems to document care and improve clinic workflows.
• Collaborate with providers, LPNs, CMAs, and hospital staff to ensure safe, high-quality care.
• Participate in chronic care management and quality improvement initiatives.
