Job summary

TheKey is seeking an RN Client Success Manager to oversee in-home care quality and client plans. You will conduct initial assessments, re-assessments, and care plan tailoring to promote independence and safety.

You’ll partner with the care team to ensure excellent client experiences and regulatory compliance, while supporting long-term client retention. Responsibilities include ensuring up-to-date documentation, coordinating with internal/external teams, and acting as a quality-control expert to

Qualifications

• Current RN license in the state of employment.
• BSN preferred.
• 5 years experience in health care, elder care, social work, or related industry preferred.
• Excellent customer service and conflict resolution skills.
• Computer proficiency and ability to document accurate and timely notes in systems related to client visits.
• Current driver’s license and proof of insurance.

Responsibilities

• Manage the day-to-day client relationship and delivery of quality care.
• Identify holistic approaches to improve client care and experience.
• Conduct in-person nursing initial and reassessments and develop Care Plans.
• Ensure care plans are up-to-date and compliant with regulations.
• Collaborate with internal and external customers through regular meetings.
• Act as subject matter expert for quality control and assurance.
