TheKey seeks a Client Success Manager to oversee a portfolio of aging-at-home clients, coordinating tailored care plans with families and medical professionals. Your role includes guiding caregiver teams and ensuring consistent, high-quality service across client engagements.

You will apply your health-care and coordination background, using EMR tools and strong communication to resolve issues, optimize outcomes, and support data-driven decisions in a fast-paced environment.

Qualifications

• Bachelor’s Degree in Human or Health Services or related field from an accredited university.
• 3 years experience in health care, elder care, social work, or related industry preferred.
• Excellent customer service and conflict resolution skills.
• Computer proficiency and EMR experience.
• Current driver’s license and proof of insurance.

Responsibilities

• Oversee day-to-day client relationships and delivery of holistic care.
• Conduct assessments to design service plans and build rapport with families.
• Manage and supervise caregiver teams ensuring plans are implemented.
• Proactively communicate with stakeholders to evaluate solutions and satisfaction.
• Handle escalations, ensure regulatory compliance, and identify growth opportunities.
• Manage Start of Care process for high-quality client experiences.
• Conduct in-person re-assessments and quality assurance visits.
