Job summary

You will conduct in-person assessments, collaborate with families, and drive ongoing care improvements while coordinating with the care team to keep plans accurate and up-to-date.

Ideal candidates have strong customer service skills, 3+ years in health-related fields, and a valid driver’s license.

Qualifications

• Bachelor's degree or equivalent work experience in health services.
• Excellent customer service and conflict resolution skills.
• Proficient in documenting client visits in care systems.
• Valid driver's license and proof of insurance.
• Preferred 3 years in healthcare or elder care.

Responsibilities

• Manage day-to-day client relationships and ensure delivery of quality care.
• Perform in-person assessments and develop care plans with families.
• Coordinate with care team and document updates for quality assurance.
• Communicate with clients, families, and internal/external stakeholders regularly.
• Review and adjust care plans, caregiver assignments, and billing accuracy.
