Job summary

TheKey is seeking an RN Client Success Manager to ensure delivery of high-quality care and accurate care plans. You will reassess clients, monitor care, and work with families to support independence and well-being. This role emphasizes compliance, team collaboration, and continuous improvement in client outcomes.

This position requires active RN licensure, strong communication, and the ability to travel and document care plans accurately for a diverse client base across North America.

Qualifications

• Current Registered Nurse license in the state of employment; continued employment is contingent upon maintaining active license in good standing; BSN preferred.
• 5 years experience in health care, elder care, social work, or related industry preferred.

Responsibilities

• Manage the day-to-day client relationship and delivery of quality care.
• Identify holistic approach to client care with focus on quality of life and independence.
• Conduct in-person nursing initial and reassessments and quality assurance visits with home safety assessments and care plans.
• Ensure care team is up-to-date with information and care plans documented and updated.
• Collaborate with internal/external partners; attend regular meetings to discuss client needs and outcomes.
• Act as subject matter expert for quality control and assurance; ensure compliance with state regulations.
