TheKey of California, LLC is seeking an experienced RN Client Success Manager to maintain high-quality client care plans, perform initial assessments and ongoing reassessments, and partner with a care team to ensure excellent service. The role emphasizes strong client relationships and thorough documentation to support long‑term care outcomes.

Qualifications

• Current Registered Nurse license in the state of employment; continued employment is contingent upon maintaining active license in good standing; BSN preferred.
• 5 years experience in health care, elder care, social work, or related industry preferred.
• Excellent customer service and conflict resolution skills.
• Computer proficiency and ability to document accurate and timely notes in systems related to client visits.
• Current driver’s license and proof of insurance

Responsibilities

• Manage the day‑to‑day client relationship and delivery of quality care.
• Identify and focus on a holistic approach to client care with a focus on quality of life and improvements.
• Conduct in‑person nursing initial and reassessments and quality assurance visits with tailored Care Plans.
• Utilize interpersonal skills to build rapport with the client and their support system.
• Ensure care team is up‑to‑date and care plan is thoroughly documented and updated
• Collaborate with internal/external customers through regular meetings and communication
• Consult with client regarding needs and objectives, and evaluate potential solutions
• Act as subject matter expert for quality control and quality assurance
• Ensure compliance with state regulations and company guidelines
• Support cross referrals and opportunities to grow business
• Assist with on‑call and after‑hours support as needed
• Additional duties as assigned
