Job summary

myPlace Health is seeking a CDI Specialist to ensure accurate, clinically specific provider documentation for the elderly population served under the PACE model. You will partner with physicians and care teams to improve CMS-HCC capture and sustain documentation quality.

The role requires an active CA RN or NP license, 3+ years in outpatient settings, strong analysis, and proficiency with CDI tools and EHRs. Competitive salary and benefits accompany a collaborative, mission-driven environment.

Qualifications

• Active, unrestricted CA RN or NP license.
• Minimum 3 years clinical outpatient/ambulatory experience.
• Ability to interpret and synthesize complex clinical data.
• Foundational knowledge of ICD-10-CM coding and documentation.
• Strong written and verbal communication skills with clinicians.
• Proficiency with electronic health record systems and workflows.
• Ability to manage a structured chart-review workload independently.
• Attention to detail, analytical thinking, clinical judgment.
• Experience in CDI, risk adjustment, or documentation improvement preferred.
• Familiarity with Medicare Advantage, PACE, capitated/value-based models.
• Certification (CRC/CDEO/CDS/O) preferred; willingness to obtain within 12 months.

Responsibilities

• Identify documentation gaps and CMS-HCC opportunities through chart reviews.
• Synthesize EMR data, labs, imaging, meds, and notes to support accurate documentation.
• Track disease progression and patient complexity trends.
• Formulate provider queries with clinical evidence for clarification.
• Collaborate with physicians and care teams to improve documentation quality.
• Participate in interdisciplinary meetings to surface opportunities in real time.
• Prioritize daily chart reviews based on risk and organizational priorities.
• Maintain CDI metrics and participate in reporting and audits.
• Develop provider education and CDI resources aligned with CMS standards.
• Elevate documentation issues to CDI program leadership.
