Job summary

myPlace Health is seeking a Clinical Documentation Integrity (CDI) Specialist to ensure accurate, complete provider documentation across our PACE participant population. You will review records, identify gaps, and collaborate with physicians and care teams to improve CMS-HCC capture and documentation quality.

Qualified candidates hold an active CA RN or NP license with 3+ years of outpatient clinical experience. CDI certification is preferred; bilingual Spanish is a plus.

Qualifications

• Active, unrestricted RN or NP license in California.
• Minimum of three years of clinical experience in outpatient, primary care, ambulatory care, or comparable setting.
• Foundational knowledge of ICD-10-CM coding principles and their relation to documentation.
• Strong written and verbal communication skills.
• Proficiency with EHR systems and digital workflows.
• Experience in CDI, risk adjustment, HCC coding preferred.
• Spanish bilingual is a plus.

Responsibilities

• Conduct chart reviews to identify documentation gaps and CMS-HCC opportunities.
• Synthesize information from EMR, labs, imaging, and notes to support accurate documentation.
• Collaborate with physicians and care teams to improve documentation quality.
• Prepare physician queries and support provider clarification without directing care.
• Track patterns and prepare reporting for program monitoring and audits.
