Job summary

myPlace Health is seeking a CDI Specialist to support accurate provider documentation for the PACE program. This role partners with physicians, nurse practitioners, and care teams to ensure documentation reflects clinical complexity and disease burden across the participant population.

The position requires an active California RN or NP license, at least three years of outpatient clinical experience, and knowledge of ICD-10-CM coding principles.

Qualifications

• Active, unrestricted RN or NP license in California.
• Minimum of three years of clinical experience in outpatient settings.
• Strong ability to interpret and synthesize complex clinical data across multiple sources.
• Foundational knowledge of ICD-10-CM coding principles and their relation to documentation.

Responsibilities

• Conduct prospective, concurrent, and retrospective chart reviews to identify documentation gaps and CMS-HCC opportunities.
• Synthesize data from EMR, labs, imaging, medications, and outside records to support accurate documentation.
• Identify disease progression and patient complexity to improve documentation of illness severity.
• Formulate compliant provider queries with objective clinical evidence.
• Collaborate with physicians and care teams to improve CMS-HCC capture and documentation quality.
• Participate in interdisciplinary meetings to surface documentation opportunities in real time.
