Westerly Rehabilitation and Healthcare Center is hiring an RN Case Manager in Rhode Island. The role focuses on care coordination and clinical decision support to ensure continuity across the care continuum.

The ideal candidate will have ADN with RN licensure and experience in case management or direct patient care, plus strong communication and regulatory knowledge. This is a full-time position with benefits and daily pay options.

Qualifications

• RN with ADN is required; BSN preferred.
• Experience in case management or utilization review is preferred.
• Direct patient care experience is strongly preferred.
• Strong communication, negotiation, and problem-solving skills.
• Solid understanding of healthcare regulations and care coordination principles.

Responsibilities

• Coordinate and manage care for patients with chronic, acute, or complex medical conditions across the care continuum.
• Conduct comprehensive patient assessments to identify physical, psychosocial, and environmental needs.
• Develop, implement, and evaluate individualized care plans with interdisciplinary teams.
• Monitor progress toward goals and adjust plans based on outcomes.
• Advocate for residents by communicating needs, goals, and preferences.
• Facilitate communication between patients, families, physicians, and providers.
• Coordinate medical appointments, referrals, transportation, and access to resources.
• Ensure efficient utilization of resources while maintaining quality care.
• Provide education to patients and families regarding diagnoses and care planning.
