Job summary

Women & Infants Hospital is seeking an RN Case Manager to coordinate clinical care across the full continuum for a designated patient population. The role emphasizes discharge planning, care coordination, and resource management to deliver quality, cost-effective outcomes.

The RN Case Manager collaborates with Medical, Nursing, and Ancillary teams to optimize care in the most appropriate setting, while meeting licensing and Certification requirements.

Qualifications

• Must hold and maintain Rhode Island RN license.
• 3–5 years of related clinical or Case Management experience.
• Certification in Case Management (CCM/ACM) earned within 24 months of employment (part-time prorated).
• Ability to communicate with physicians and multidisciplinary team to document cases and influence care decisions.
• Proficient in multiple computer systems (EMR, MS Office, etc.).
• Participation in weekend and holiday coverage rotations.

Responsibilities

• Coordinate clinical care for a designated patient population across the continuum of care.
• Perform discharge planning and care coordination to ensure optimal outcomes.
• Engage with Medical, Nursing and Ancillary staff to support quality-based, cost-effective care.
• Support utilization review and resource management activities as assigned.

The RN CM interacts with Medical, Nursing, and Ancillary department staff to facilitate quality-based, cost-effective patient outcomes and ensure care is provided in the most appropriate setting.

Duties & Responsibilities

• Transition of Care Planning
• Utilization Review
• Point of Entry and Emergency Department Duties, when applicable per assignment
• Utilization Review for POE/ED RN Case Manager
• Resource Allocation for POE/ED RN Case Manager
• Quality and Compliance
• Performs other duties as required

Requirements

Must have and maintain license as Registered Nurse in Rhode Island. Minimum of 3 to 5 years related clinical and/or Case Management experience required. As of 1/1/16, Certification in Case Management (CCM or ACM) earned within 24 months of employment, prorated for part-time staff. Ability to effectively communicate with and gain cooperation of resident and attending physicians with regards to documentation needs and to effectively influence all members of the interdisciplinary care team during difficult cases to achieve appropriate resolutions. Must have effective written communication. The ability to interpret medical data as documented. Utilization of level of care management, quality management, discharge planning, basic knowledge of third party payer or case management knowledge preferred. Knowledge and training in multiple computer systems required to perform position responsibilities (i.e. EMR, Microsoft Office, etc.) Participation in weekend and holiday coverage rotation. Flexibility to cover as needed.
