Job summary

The Independent Health Association is seeking a registered nurse Case Manager to coordinate care for patients across the continuum. You will assess, plan, implement and evaluate care while collaborating with physicians, social workers and payors to optimize outcomes.

Ideal candidates have an active RN license in an NLC state, CCM certification, and at least three years of clinical RN experience in med/surg or ambulatory settings. Strong communication and problem-solving skills are essential.

Qualifications

• Active RN license in an NLC state (unrestricted).
• CCM certification required; if not held, obtain within 3 years.
• Minimum 3 years of clinical RN experience in med/surg or ambulatory care.
• Experience in managed care is a plus.
• Excellent written and verbal communication skills.
• Ability to interact effectively with physicians and the care team.
• Flexibility in work schedules and assignments.

Responsibilities

• Assess the patient's broad needs through social and medical history.
• Develop a care plan considering physical and psychosocial needs and cost factors.
• Continuously evaluate care plans and monitor quality within the team.
• Ensure compliance with regulatory and privacy standards and manage documentation.
• Establish professional relationships with the inter disciplinary team.
• Advocate for patients and identify potential complications early.
• Understand ADA and disability-related legislation and apply accordingly.
• Identify community resources to support patient recovery.
• Communicate available services to patients and assist in accessing them.
• Coordinate with payors and providers to evaluate service quality and cost.
• Explore alternative levels of care when appropriate and implement changes.
• Document the plan of care in a timely manner and maintain records.
• Participate in training and committee activities related to Case Manager role.
