Job summary

CVS Health in Wisconsin is seeking a Community Care Case Manager to lead collaborative care for members, coordinating benefits, clinical coverage, and programs across providers and community partners. The role requires an RN license, 5+ years clinical experience, and 2+ years discharge planning or home health care coordination experience; bilingual and CCM certification preferred.

Qualifications

RN license required.
Minimum 5+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
Minimum 2+ years discharge planning and/or home health care coordination experience.

Responsibilities

Facilitate the delivery of appropriate benefits and/or healthcare information to determine eligibility for benefits and promote wellness activities.
Use a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet individuals' health needs.
Collaborate with members, providers and community organizations to address comprehensive health and social determinant needs.
