Job summary

CVS Health is seeking a Community Care Case Manager to elevate patient care through collaborative assessment, planning, facilitation and advocacy. You will coordinate with members, providers and community resources to meet health and social needs, promoting wellness and timely return to work.

This role emphasizes care coordination and individualized support across the care continuum. The position requires an RN license, 5+ years of clinical experience with Medicare members and at least 2+ years

Qualifications

• RN license in good standing; compact license may be required.
• 5+ years of clinical experience; preference for diabetes, CHF, CKD, post-acute, hospice or palliative care with Medicare.
• 2+ years of discharge planning and/or home health care coordination.

Responsibilities

• Facilitate benefits/health information to determine eligibility and promote wellness.
• Coordinate care with members, providers, and community resources across the continuum.
• Assess, plan, facilitate, and advocate for services to improve outcomes.
