Qualifications

• RN license required; compact license preferred where applicable.
• 5+ years clinical experience with Medicare populations.
• 2+ years discharge planning or home health coordination.

Responsibilities

• Coordinate assessment, planning, facilitation, care coordination and advocacy.
• Collaborate with members, providers and community resources.
• Promote wellness activities and monitor outcomes.
• Support timely return-to-work through comprehensive planning.
