Job summary

CVS Health is seeking a Community Care Case Manager to elevate patient care by coordinating benefits, healthcare information, and services for members nationwide. You will use a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy to meet individuals' comprehensive health and social needs.

Requires RN license, 5+ years of clinical experience (diabetes, CHF, CKD, post-acute, hospice or palliative care), and 2+ years in discharge planning or home

Qualifications

• RN compact license required.
• 5+ years clinical experience; Medicare-focused preferred.
• Experience in diabetes, CHF, CKD, post-acute, hospice or palliative care.

Responsibilities

• Assess, plan, facilitate, coordinate, evaluate and advocate for members' health needs.
• Coordinate benefits and services with providers, community organizations and families.
• Promote wellness activities and support timely return to work.
