Job summary

CVS Health in Pennsylvania is seeking a Community Care Case Manager to help manage member care across the continuum. You will assess, plan, coordinate, and advocate for services with providers, families, and community resources to improve outcomes and efficiency.

The role requires an RN license, 5+ years of clinical experience (diabetes, CHF, CKD, post-acute, hospice), and 2+ years in discharge planning or home health coordination. Multistate licensure and bilingual abilities are a plus.

Qualifications

• RN license required.
• 5+ years of clinical experience (diabetes, CHF, CKD, etc.) with Medicare members.
• 2+ years in discharge planning and/or home health care coordination.

Responsibilities

• Facilitate benefits eligibility and coordinate health services to meet member needs.
• Collaborate with members, providers and community organizations to improve outcomes.
• Assess, plan, implement and evaluate care to promote quality and cost-effective results.
