Qualifications

• RN compact license required.
• 5+ years clinical practical experience with Medicare members.
• 2+ years discharge planning and/or home health care coordination experience.
• 2+ years Case Management experience preferred.

Responsibilities

• Coordinate assessment, planning, facilitation, care coordination, evaluation, and advocacy for members and families.
• Collaborate with members, providers, and community organizations to address comprehensive health needs.
• Promote access to benefits and wellness activities to support timely return to work.
