Qualifications

• RN license required.
• Minimum 5+ years clinical practical experience (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members.
• Minimum 2+ years discharge planning and/or home health care coordination experience.

Responsibilities

• Coordinate care management for Medicare members across settings.
• Assess, plan, facilitate, care coordinate, evaluate and advocate for options and services.
• Promote benefits and wellness activities and ensure timely return to work.
• Collaborate with members, providers and community organizations to address health and social needs.
