Qualifications

• RN Compact license required.
• 5+ years clinical experience with Medicare members.
• 2+ years discharge planning or home health care coordination.
• 2+ years Case Management experience preferred.

Responsibilities

• Assess, plan, facilitate and coordinate care for individuals and families to meet health needs.
• Advocate for options and services to promote quality, cost-effective outcomes.
• Collaborate with members, providers and community organizations to address health and social determinants of care.
• Promote wellness activities and timely return to work.
