Job summary

MyMichigan Health seeks a Care Manager to provide care management and coordination for adult and pediatric patients in PCP offices in Chesaning and Birch Run. You will lead the care management process within the team, collaborating with PCPs and specialists to develop individualized plans and monitor progress.

The role requires RN licensure with experience in case management, chronic disease management, and health plan/payer knowledge.

Qualifications

• Associates degree required; BSN preferred.
• 2 years of case management experience preferred.
• 3 years of experience with adult medicine/pediatric patients in primary care/ambulatory care, home health, SNF, or hospital setting.

Responsibilities

• Collaborates with PCP, patient, and care team to develop a comprehensive care plan and monitor response.
• Identifies high-risk patients within the practice per PCP referral and risk stratification.
• Implements interventions based on risk stratification and guidelines.
• Assesses ongoing patient/family health, educational, and psychosocial needs.
• Provides self-management support to empower patient/family.
• Implements systems of care for close monitoring of high-risk patients.
