Job summary
w3r Consulting is looking for a Case Manager RN for Blue Cross Blue Shield of Michigan. As the primary contact for members, you will coordinate care among various healthcare professionals, assess member needs, and develop individualized care plans. Candidates should have a nursing diploma or associate degree, at least 3 years of clinical nursing experience, and an active RN license. Strong communication and Microsoft Office skills are required. Position is based in Detroit, Michigan and offers a 12-month contract.
Qualifications
Nursing Diploma or Associate Degree in Nursing (BSN preferred).
Minimum 3 years of clinical nursing experience (acute/post-acute/community setting).
Strong communication, organizational, and critical thinking skills.
Proficiency with Microsoft Office tools.
Responsibilities
Coordinate care across a multidisciplinary team including social workers, dietitians, pharmacists, and medical directors.
Assess members’ clinical, psychosocial, and support needs.
Develop, implement, and monitor individualized care plans.
Identify care gaps, address barriers, and improve health outcomes.
Connect members with community and healthcare resources.
Provide education on self-management, medications, and overall health.
Support care transitions and ensure continuity of care.
Document all interactions accurately and maintain compliance with guidelines.
