Your role includes navigating Medicare/Medicaid guidelines and driving safe transitions across the care continuum. Ideal candidates hold an RN or LMSW with MI licensure and 3+ years of clinical experience, with CCM/ACM certifications preferred.

Qualifications

• Bachelor's degree in Nursing or Master's in Social Work required.
• Three years of clinical experience preferred.
• Excellent communication and documentation skills required.
• Strong knowledge of Medicare/Medicaid regulations and case management.
• Ability to coordinate discharge planning and community resources.

Responsibilities

• Identify patient and family needs through comprehensive assessments.
• Develop and implement strategic care plans to optimize outcomes.
• Coordinate discharge planning and transitions to home health or rehab.
• Advocate for patients and connect them with community resources.
• Collaborate with internal and external stakeholders to manage complex cases.
