Job summary

AccentCare is seeking an RN Case Manager in Michigan to manage patient care plans from admission through discharge, coordinating with nurses and CHHAs to ensure quality care. The role is full-time, Mon-Fri, 8:30am-5pm, with on-call duties and a per-point compensation structure.

Responsibilities include initial assessments, care plan development, coordinating interdisciplinary teams, and documenting progress in the chart.

Qualifications

• Must be a graduate from an approved school of nursing and licensed to practice as an RN in Michigan.
• Minimum of one year of RN experience.
• Current CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient care plan.
• Complete initial patient assessment at admission with history.
• Identify nursing diagnoses and establish measurable goals.
• Coordinate interventions and communicate with physicians.
