The role focuses on preventing readmissions, coordinating care across physicians and teams, and ensuring timely, appropriate levels of care with accurate medical necessity criteria.

Qualified candidates will have 3–5 years of clinical experience and may hold or pursue ACM certification.

Qualifikationen

• Three to five years clinical experience.
• Certification in case management preferred.
• Proficiency with Microsoft Windows and health IT systems.
• Ability to educate physicians and staff on care management concepts.

Aufgaben

• Work with healthcare team, patient, family to transition to appropriate level of care; advocate and negotiate for patient needs.
• Collaborate with care management team on admission, discharge planning, and regulatory standards.
• Lead daily care coordination meetings and document key information and next steps.
• Facilitate discharge planning, including extended care placements and home care arrangements.
• Determine necessity for second-level review and negotiate patient status with payer.
• Analyze admission data to ensure quality care and appropriate location on daily basis.
