Responsibilities

• Collaborate with physicians, nurses, social workers, and post-acute providers to ensure timely interventions.
• Monitor progress, address barriers, and optimize care pathways.
• Lead discharge planning and ensure smooth transitions across care levels.
• Participate in quality improvement initiatives and comply with regulations.
