AccentCare, Inc. is seeking an RN Case Manager to oversee care plans from admission to discharge, coordinating with nurses and clinicians to ensure quality patient care. The role involves direct and indirect patient care, supervision of nursing staff, and collaboration across disciplines.

Base pay ranges from $87,000 to $102,000 per year, paid on a per-point basis with a one-time sign-on bonus of $5,000. Schedule is Monday–Friday, 8:00 a.m.–5:00 p.m., with benefits and opportunities for

Formación

• Graduate from an approved school of nursing and licensure as an RN in the state of operation.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsabilidades

• Assess the nursing needs of assigned patients and manage the care plan.
• Complete initial patient assessment at admission with history.
• Identify appropriate nursing diagnoses.
• Evaluate medications for effectiveness, adverse reactions, allergies, and contraindications.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physician of assessments and recommended interventions.
• Implement nursing interventions within Nurse Practice Act and care plan.
• Re-evaluate patient needs as required.
• Document assessments, progress, and discharge notes timely.
• Notify physician and staff of changes in patient condition.
