Job summary

AccentCare, Inc. is seeking an RN Case Manager to oversee patients' care plans from admission through discharge, coordinating with staff to ensure quality care and optimal outcomes for each patient.

The role combines direct and indirect patient care, requires an RN license in the operating state, and offers a $5,000 sign-on bonus with a pay range of $87,000 to $102,000 per year for a Monday–Friday schedule in the Edina/Golden Valley area.

Qualifications

• Graduate from an approved school of professional nursing and currently licensed to practice as an RN in the state of operation.
• Minimum of one year of experience as an RN.
• Current CPR certification required.

Responsibilities

• Assess nursing needs and manage the patient's care plan from admission through discharge.
• Complete initial patient assessment, including patient history.
• Select appropriate nursing diagnoses and monitor therapies for safety and effectiveness.
• Review medications for interactions and adverse reactions.
• Set realistic, measurable goals aligned with diagnosis.
• Inform physicians of assessments and interventions; document progress.
• Implement nursing interventions and re-evaluate as needed.
• Document assessment, progress and discharge notes timely.
• Communicate patient changes to physicians and team.
