AccentCare, Inc. in Minnesota seeks an RN Case Manager to oversee patient care plans from admission through discharge, coordinating with RNs, LPNs, and CHHAs to ensure quality care. This full-time role includes a sign-on bonus of $5,000 and pay in the range of $87,000 to $102,000 per year, with a Monday–Friday schedule 8:00 a.m.

Qualifications

• Graduate from an approved school of professional nursing and licensed to practice as an RN in the state of operation.
• Minimum of one year of RN experience.
• Current CPR Certification required.

Responsabilités

• Assessment of nursing needs and overall management of the patient\'s care plan.
• Complete initial patient assessment on admission, including patient history.
• Select appropriate nursing diagnosis.
• Assess medications for potential interactions, adverse reactions, allergies, and contraindications.
• Establish realistic, measurable goals aligned with diagnosis.
• Inform physician of assessment results and recommended interventions.
• Implement interventions within Nurse Practice Act scope.
• Re-evaluate patients\' nursing and clinical needs as required.
• Document assessment, progress, discharge notes, and required paperwork promptly.
• Inform physician and staff of changes in patient condition and needs.
