AccentCare, Inc. in Minnesota is seeking an RN Case Manager to manage patients' care plans from admission through discharge and to coordinate with staff nurses, LPNs, and CHHAs to deliver a quality plan of care.

This full-time position follows a Monday–Friday schedule, 8:00 a.m. to 5:00 p.m., with a $5,000 sign-on bonus. Pay is per-point and the posted compensation reflects an annual estimate for the role.

Qualifications

• RN license in the state of operation
• Minimum of one year experience as an RN
• Current CPR certification

Responsabilités

• Assess nursing needs and manage the patient's care plan
• Complete initial patient assessment upon admission
• Select appropriate nursing diagnoses
• Assess medicines for potential drug interactions or adverse reactions
• Establish measurable goals aligned with diagnosis
• Inform physician of assessments and interventions
• Implement nursing interventions per plan and licensure
• Re-evaluate nursing and clinical needs as needed
• Document assessments, progress, and discharge notes timely
• Inform physician and staff of changes in patient condition
