Research Medical Center is seeking an RN Case Manager to support patients and families through their care journey. You will coordinate services, develop individualized plans, and partner with physicians, nurses, and leaders to ensure safe transitions and clear communication.

Your role emphasizes discharge planning, education for patients and families, and collaboration with the Rehab Program Director to deliver consistent, high-quality care across the continuum.

Qualifications

• Must hold an RN license in good standing.
• Diploma or degree as Registered Nurse.
• Experience in case management or care coordination preferred.

Responsabilités

• Guide patients and families through care transitions and discharge planning.
• Coordinate education for patients and families on treatment plans and rights.
• Develop individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and document findings clearly.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote compassion, respect, and excellence in all interactions.
