Job summary

Research Medical Center is seeking an RN Case Manager to support patients and families through the care journey. You will coordinate services, connect resources, and develop care plans with physicians and the rehab team to ensure safe transitions and high-quality care.

You will guide patients through program orientation, discharge criteria, and insurance benefits, while promoting compassion and excellence in every interaction.

Qualifications

• 1 year of RN case management experience required.
• Strong communication and organization skills.
• Ability to coordinate care across multidisciplinary teams.

Responsibilities

• Guide patients and families through orientation, Medicare/insurance benefits, discharge criteria, and patient rights.
• Coordinate education for patients, families, and caregivers to support treatment planning and family conferences.
• Develop and implement individualized treatment plans reflecting patient strengths and recovery goals.
• Complete psychosocial assessments and discharge planning evaluations; document findings and inform the care team.
• Collaborate with Rehab Program Director and interdisciplinary team to ensure seamless, patient-centered care.
• Promote HCA Healthcare values of compassion, respect, and excellence in every interaction.
