Job summary

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. The Case Manager, RN guides participant care as a crucial voice on the interdisciplinary team (IDT).

Reporting to clinical leadership, the Case Manager, RN delivers and oversees all aspects of care, including conducting required PACE assessments, developing and following personalized participant care plans, and supporting transitions of care coordination.

Qualifications

• Graduate of an accredited school of nursing; BSN preferred.
• Unencumbered California RN License.
• Current Basic Life Support (BLS) / First Aid Certification required.
• Two years of nursing experience with frail/elderly population preferred.
• CPR and First Aid certification within 180 days of hire preferred.

Responsibilities

• Conduct clinic nursing assessments to determine participant needs, preferences, and goals, and actively participate in IDT meetings to develop participant care plans.
• Deliver and document nursing interventions as agreed upon in participants’ care plans, including maintaining a safe environment, responding to physician orders, and administering medications.
• Collaborate with the WelbeHealth PACE Physician and IDT to coordinate participant care needs, and communicate with participants and families to revise care plans.
• Instruct participants, families, and caregivers regarding the disease process, self-care techniques, and prevention strategies.
