Qualifikationen

• Current LPN license in Kansas is required.
• Graduation from an accredited practical nursing program.
• Ability to provide nursing care within LPN scope under appropriate supervision.
• Strong observation, communication, and documentation skills.
• Ability to collaborate effectively with an interdisciplinary care team.
• Basic computer skills and experience with electronic health records.

Aufgaben

• Provide direct nursing care to participants within the clinic setting as part of an interdisciplinary care team.
• Monitor participants’ health status by checking vital signs and reporting changes to the supervising RN or provider.
• Assist in carrying out individualized care plans and perform basic clinical tasks within scope of practice.
• Administer medications and treatments as ordered; document all care in the medical record.
• Coordinate appointments, support clinical procedures, and communicate with team members to ensure comprehensive care.
• Maintain accurate documentation in the medical record.
