Job summary

Midland Care Connection in Topeka, KS is seeking an RN Clinic Nurse to join its PACE program. The role provides hands-on nursing care, health assessments, medication administration, and coordination with an interdisciplinary team to support older adults at home and in clinic settings.

You will educate participants and caregivers, respond to urgent health concerns, document care accurately, and contribute to personalized plans that keep seniors independent.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of Kansas
• Graduate of an accredited nursing program
• Current CPR certification
• Experience providing nursing care to older adults or individuals with complex medical needs
• Strong assessment, critical-thinking, and clinical decision-making skills
• Ability to communicate effectively with participants, families, caregivers, and members of the interdisciplinary team
• Ability to accurately document patient care and maintain confidentiality
• Ability to work collaboratively as part of an interdisciplinary team
• Commitment to providing compassionate, participant-centered care

Responsibilities

• Provide hands-on nursing care to PACE participants in the clinic setting
• Assess participants' health needs and monitor changes in their conditions
• Administer medications and treatments
• Respond to urgent health concerns and coordinate with the interdisciplinary care team
• Educate participants and caregivers on health management and care plans
• Ensure safe, personalized care and documentation of all care activities
