Home Health Care, Inc. is seeking an experienced RN Case Manager in Edina, MN to oversee a home health caseload within the Southwest Minneapolis and inner metro area.

You will coordinate care, perform assessments, and educate patients and families while working with an interdisciplinary team. You will manage transitions from admission to discharge, ensure quality outcomes, and maintain thorough documentation using HCHB.

Qualifikationen

• Active Minnesota RN license is required.
• Three or more years of RN experience in nursing.
• OASIS experience is highly valuable.
• Home health experience preferred.
• Excellent organization and time-management skills.

Aufgaben

• Oversee an assigned home health patient caseload.
• Complete OASIS Start of Care, Recertification, and Discharge assessments.
• Develop and update individualized plans of care.
• Monitor patient progress throughout the episode of care.
• Educate patients and caregivers on diagnoses, medications, treatments, and recovery.
• Coordinate with physicians and therapists and other team members.
• Complete timely documentation using Homecare Homebase (HCHB).
