Qualifications

Responsibilities

• Assumes responsibility for caseloads in specialty areas.
• Conducts initial assessment to obtain physical, social and environmental data.
• Establishes plan of care per physician orders and patient needs.
• Provides direct patient care and adjusts nursing care as needed.
• Communicates with physicians to update plan of care and ensure team coordination.
• Facilitates discharge planning and continuity of care after discharge.
• Maintains accurate clinical and patient records per guidelines.
• Participates in quality improvement activities and audits.
• Demonstrates age-specific knowledge and care planning.
