Job summary

Qualifications

• Registered Nurse license required in state of practice
• BLS for Healthcare Providers certification within 6 months unless exempt
• Valid driver's license
• Typically 3 years of clinical nursing experience requested
• Must be able to provide age-appropriate care and assess developmental needs

Responsibilities

• Assumes responsibility for caseloads consisting of patients in specialty areas.
• Conducts initial assessment to obtain thorough physical, social and environmental data.
• Establishes the plan of care in accordance with physician orders and patient needs.
• Provides direct patient care and adjusts nursing care processes as needed; identifies problems and evaluates outcomes.
• Provides ongoing assessment of patient response to treatments and educational needs; revises plan of care as indicated.
• Completes documentation including visit assessments and orders accurately and promptly.
• Communicates with physicians to confirm and update patients' plan of care; keeps team informed.
• Facilitates discharge planning and coordinates continuation of care after discharge.
• Participates in chart audits and quality improvement activities.
• Maintains patient records per agency, state, and federal guidelines.
• Demonstrates age-appropriate care across life span as per department standards.
