Eastern Idaho Regional Medical Center in Idaho Falls is seeking a Case Manager to guide patients and families through the care journey, coordinating services and developing care plans with physicians, nurses, and leaders to ensure safe transitions and consistent quality across the continuum of care.

You will support program orientation, explain rehabilitation philosophy and Medicare/insurance benefits, and coordinate education and discharge planning with the care team to optimize patient

Qualifikationen

• RN license required, valid in Idaho.
• BCLS certification within 7 days of start date.
• Associate Degree required.

Aufgaben

• Guide patients and families through program orientation and discharge criteria.
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning.
• Develop and implement individualized treatment plans reflecting patient needs and goals.
• Complete psychosocial assessments and communicate needs across the care team.
• Collaborate with Rehab Program Director and interdisciplinary team for patient-centered care.
• Promote compassionate, respectful, high-quality care in line with HCA values.
