Qualifications

• Basic Cardiac Life Support within 7 days of employment start date
• RN - Registered Nurse
• Associate Degree required

Responsibilities

• Guide patients and families through program orientation and discharge planning
• Coordinate education for patients, families, and caregivers to participate in treatment planning
• Develop individualized treatment plans reflecting patient needs and recovery goals
• Complete psychosocial assessments and discharge planning evaluations and document findings
• Collaborate with Rehab Program Director and interdisciplinary team for seamless, patient-centered care
• Promote values of compassion, respect, and excellence in interactions with patients and families
